This is a SAVE point. It's the manifestation of your own DETERMINATION.

A SAVE Point (JP: セーブポイント) is a place where the protagonist can SAVE the game. Interacting with a SAVE Point fully restores their HP and usually expresses the protagonist's current thoughts and determination. By default, a total of 30 SAVE Points can be found in the underground, with a few more added or altered depending on the route.

On the Genocide Route, SAVE Points instead display a kill counter that indicates the number of monsters left to kill before the kill counter exhausts in an area with the message "<number> left." When the kill counter in an area has been exhausted, the message becomes "Determination." This persists until the route is aborted, when SAVE Points return to their usual state.

Autosave locations

The game occasionally creates hidden autosaves. These saves are reloaded on Game Over, but not when the game is closed and reopened. Regular SAVE Points also override the autosave file.

Trivia

• Snowdin's "Box Road" and Waterfall's "Crystal" SAVE Point areas has a chance of a location appearing nearby based on the fun value. It is the Sound Test room and the fake hallway respectively.
• The "Bad Opinion Zone" SAVE Point references Alphys updating her UnderNet status to express her disdain for Mew Mew Kissy Cutie 2.
• In v1.00, the SAVE Point message for Snowdin on the Genocide Route, "That comedian..." appeared even after Snowdrake was killed. According to Toby Fox, the message's frequency was a bug and was fixed in version 1.001.
