Ok, so.
Id recommend if you’re editing sprites, try to get it into a pixel by pixel setting. smaller pixels on larger pixels look… weird.
as well, for original stuff, don’t outline with black. if you HAVE to outline, use gray (light is good.) Outline with the color of the thing, It might look bad at first, but TRUST it becomes better

I’m sorry if this sounds too harsh, but this isn’t really pixel art, this is just putting rectangles onto Sans’s sprite
All of the squares need to be the same size and in a grid
Like look at the official Undertale sprites
Like the other person said it’s best to get the image at a 1:1 scale and edit pixel by pixel directly
Some people use Pixilart for this

This is a personal preference thing. Lots of fairly good-looking pixel games have black outlines to some extent, like Mother 3, Lisa: The Painful, or Deltarune. How you want to really do outlines comes down to what you think will look good, but it is fair to experiment with that to actually get an IDEA of what will look good.

Deltarune does this style of outlines, so it works for Undertale/Deltarune pixel art.

For The Red eyes - Its Ruins Murder/Dust Sans. so he has Red Eyes.
For The Teeth - It Just Didnt Make Sense For Sans To Be Smiling If He’s Killing Monsters.
