The Titan is the boss of the Third Sanctuary. The boss theme for the sanctuaries in Earthbound is called Sanctuary Guardian. It’s a disguised Earthbound reference.
(Sanctuary Guardian isn’t technically the official name for the song or the group of bosses, but it’s such a prevalent fan name there’s no way Toby hasn’t heard it before. )

On one hand, we already do that for Undertale's Another Medium (just under Creation and Development instead of Trivia).
On the other hand, this feels more speculative than Another Medium somehow.
Actually, Another Medium's name origin might not be less speculative after all. I only thought that way because I have been hearing about it since I was a child.
Another Medium is pretty solid, in my opinion. It’s an adaptation of a song he made for a Land in Homestuck, which reside in a space called the Medium, and I don’t know what else it could mean.
