First of all no? He's not photorealistic? He's 3D sure but photorealistic has a different meaning.
Second of all, imagine you spend your whole life in the style of deltarune (that being 2D pixel art) and the only knowledge you have about how humans look os either from your friend (who also looks like 2D pixel art) or from a book with some photos of also seemingly 2D pixel art humans. And suddenly you see a photorealistic human face. Wouldn't that freak you out at least a bit?

No I don't but my point stands that a photorealistic human face would be viewed as something weird and unnatural in this world.
Even if they aren't 2D, the few humans we've seen are all stylized and not real looking at all.
If Kris, unpixelated and in the light world, looked like an actual human then what's this?
You can see Kris, in much higher detail, yet not looking that much different from the normal sprite. Therefore, it still stands to reason that the sight of a realistic human would be a shock.
